Your care pathway
Completion and aftercare
We complete the treatment when things are going well. After that, your GP takes over your care, and our door stays open for another six months.
When we complete treatment
We complete the treatment together with you when:
- your goals have been achieved;
- your medication dose is stable, if you take medication;
- you have enough tools to manage daily life.
At completion
Evaluation
What has it achieved?
You fill in the questionnaire about your symptoms again, the same one you filled in at the start. This way we see together what the treatment has achieved.Your experience
The CQI survey
With an independent questionnaire, we ask about your experiences with our care. In 2025, clients gave us an average score of 4.51 out of 5. Read client experiencesClosing letter
A letter to your GP
With your consent, your GP receives a letter with the diagnosis, the course of the treatment, the medication you have been settled on and our advice for what comes next.Medication
Handing over prescriptions
Because the letter states which medication you have been settled on, your GP can take care of your repeat prescriptions in consultation.
After completion
Your GP takes over
Straight after completion
In consultation, your GP takes care of your repeat prescriptions. If you still need support, this can be arranged, for example, through the mental health practice nurse at your GP practice (POH-ggz), basic mental health care (basis-ggz) or a targeted referral.Coming back without a new referral
Up to six months after completion
If things are going less well, we can reopen your treatment. You do not need a new referral for this. Please contact us.A new referral
After six months
You will then need a new referral from your GP to come back to us.
Are things going less well after completion?
Within six months of completion, you can come back to us without a new referral. Call or email us and we will discuss what you need.
