Your care pathway
Your care pathway, step by step
From the referral by your GP to the handover back to your GP. Our care pathway is short and intensive, and takes four to six months on average.
The five phases
Phase 1 of 5 · Registration
From referral to your first appointment
Your care pathway starts with your GP (general practitioner), who refers you to us for specialised mental health care.
Who: Your GP and our team
- ReferralYour GP or mental health practitioner sends the referral to us via ZorgDomein, the Dutch online referral system.
- ReviewOne of our practitioners reviews your referral. You will be notified by email.
- Waiting timeWe keep to the order of the waiting list. We cannot speed this up.
- SchedulingWhen it is your turn, we will contact you to schedule the appointments for the diagnostics.
Phase 2 of 5 · Diagnostics
A careful picture of your symptoms
The assessment consists of several interviews and components. No single answer or questionnaire determines the outcome.
Who: Psychologist, doctor and psychiatrist
- Information from your childhoodA parent or someone else close to you fills in a questionnaire about what you were like as a child.
- DIVA-5 interviewWith a psychologist, you discuss the features of ADHD, then and now.
- Medical intakeA doctor or nurse specialist also looks at other mental and physical conditions.
- Multidisciplinary consultationPractitioners from different disciplines jointly assess whether a diagnosis of ADHD can be made.
- Advice consultationThe doctor and the psychiatrist discuss the outcome and the advice with you. Your referrer is also informed.
Phase 3 of 5 · Treatment
A treatment plan that suits you
If you are diagnosed with ADHD, your treatment starts straight away, without a new waiting time.
Who: Your lead practitioner (regiebehandelaar) and the team
- Treatment planAt the start, you and your practitioner draw up a treatment plan together.
- PsychoeducationYou learn what ADHD is and how it affects your life. Your partner or family is welcome.
- MedicationIf you wish, we carefully find the right medication and dose for you, with regular check-ups.
- Cognitive behavioural therapyYou work on skills for planning, structure, procrastination and emotions.
- E-healthBetween appointments, you work with online modules on the Embloom platform.
Phase 4 of 5 · Completion
Completing treatment when things are going well
Treatment is completed when your goals have been achieved, your medication is stable and you have enough practical tools.
Who: You, your practitioner and your GP
- EvaluationYou fill in a questionnaire about your symptoms again, and a questionnaire about your experiences with our care.
- Discharge letterWith your consent, your GP receives a letter with the diagnosis, the course of treatment and the advice for what comes next.
- MedicationThe letter states which medication has been prescribed and adjusted, so that your GP can take over the prescriptions.
Phase 5 of 5 · Aftercare
After completion
After completion, your care returns to your GP. If things do not go so well after all, you can come back.
Who: Your GP
- GPBy agreement, your GP can then take care of your repeat prescriptions.
- Support close to homeIf you still need support, this can be arranged, for example, through your GP practice's mental health assistant (praktijkondersteuner ggz).
- Relapse within six monthsWe can then reopen your treatment without a new referral.
- After six monthsYou will then need a new referral from your GP.

What we ask of you
We measure what it achieves for you
At the start
You fill in a questionnaire about your symptoms (ROM, Routine Outcome Monitoring). That way we know where you are starting from.
Along the way
Your treatment is evaluated and discussed in a multidisciplinary consultation.
At completion
You fill in the questionnaire again, so that together we can see what the treatment has achieved.
Your experience
Through the independent CQI survey (Consumer Quality Index), we ask about your experiences with our care.
4.51/ 5
Average score out of 5 in the independent CQI survey of 2025.
4.9/ 5

Questions about the care pathway
- Four to six months on average, from the first assessment to completion.
- That depends on your health insurer. On the Waiting time page you can see the current waiting times.
- No. If you are diagnosed with ADHD, your treatment starts straight away.
- During the assessment you speak to different practitioners, because each one does a different part. Your lead practitioner is your permanent point of contact, and we aim for you to be supported by the same practitioners as much as possible.
